How to make an initial Rehab Therapy Consultation for your pet

  1. Complete a new client intake form online. You will receive an email confirmation within 48-72 hours.


     

  2. Have your primary veterinarian complete the required referral form and request for medical records, and return via email to our office.
     

  3. Once all the information has been received, you will be contacted by one of our therapists to schedule your initial consultation.

We will not be able to schedule your initial appointment until all required information has been received.

In order to receive therapy at CORE we require the following:

  1. Veterinary Referral Form filled out and signed by your primary veterinarian. This must be updated annually to continue services.


     

  2. Medical records and images pertinent to the reason for the needed therapy. 
     

  3. Completed Client Intake Form

Policies

Cancellation and No-Show Policy

We require 24 hours' notice for all appointment cancellations. Appointments canceled within 24 hours are subject to a cancellation fee for the specific scheduled service. If you miss your appointment time, the cancellation fee will also apply, or a deduction will be taken from any remaining therapy package(s).

  • Initial Consult Cancellation/Miss Fee: $100

  • Follow Up Cancellation/Miss Fee: $50

  • Hydrotherapy Cancellation/Miss Fee: $50

Late Policy

Due to the high volume of patients, we are unable to accommodate late appointments. If you are late for your appointment, you will either need to reschedule or your session will be shortened. The appointment fee will remain the same.

CORE Veterinary Rehab therapists are not permitted to diagnose or treat medical conditions, or prescribe medications for your pet.  If your pet is experiencing a medical concern, please seek out care with your primary veterinarian or local animal emergency center. Services are provided by physical therapists and certified veterinary technicians, who have received training and certification in animal rehabilitation and massage therapy through The Canine Rehabilitation Institute and The Healing Oasis. Veterinary Technicians and Assistants are under the supervision of a licensed veterinarian.
